On-line Ordering
Utilize the shopping cart feature located on each product description page by entering the quantity desired and clicking on the "Add To Cart" button. This will take you to a summary of your order. From there you can continue shopping or submit your order for processing via our secure commerce server by following the easy instructions to complete the on-line order form.

Fax Orders
Complete the same information as you would for a Postal Service Order (see below). Then fax to: 860-657-2800.

List the product names and quantities of each item that you would like (please print). You can call us at the telephone numbers listed under Direct Telephone Orders to find out the exact cost including shipping.

Enclose a check or a charge card number (including the card expiration date) and include the following information:

• Ship-To Name

• Ship-To Address

• Ship-To City, State and Zip Code (or equivalent for destinations outside the U.S.A.)

• Country and Country Code (for international orders)

• Phone Number

• E-mail Address (for on-line order confirmation and to receive occasional health bulletins)

• Visa or Master Card Number and Expiration Date (if using a charge card)

• Name on Visa or Master Card (if using a charge card)

Customs issues, if any, are your responsibility. Any customs or import duties are levied once the package reaches its destination country. Additional charges for customs clearance must be borne by the recipient; we have no control over these charges and cannot predict what they may be. Customs policies vary widely from country to country; you may want to contact your local customs office for further information.

The minimum international order is $100 USD. For international orders less than $300 USD, an additional $10 handling fee will be charged. This fee is additional to the automatically calculated shipping cost that is detailed on the online order form.

For international orders greater than $500 USD, we may be able to arrange less costly shipping than what is automatically calculated on the order form. Please contact us via e-mail at info@biomed-health.com with the details of what you would like to order.  

Payment Terms
All prices are quoted in U.S. dollars. We accept money orders, and Visa or Master Card credit cards.  

Shipment
U.S. Orders are sent US Postal Service or UPS and delivered direct to your doorstep.
International order shipping varies by the destination.
